OIC Integration Failure Audit Template

Use this template when Oracle Integration Cloud environments are creating recurring support load, weak monitoring visibility, or unclear ownership in production.

• Which integrations fail most often, and how are failure classes currently grouped?
• Is the team measuring throughput, retries, stuck transactions, and business exception patterns?
• Do support, business, and delivery teams agree on who owns each integration path?
• Are alerts tied to business impact, or only to technical signal noise?
• How are delayed escalations and partial-success flows exposed to leadership?
• What manual reconciliation work exists because monitoring and exception handling are weak?

The goal is not just to find broken integrations. It is to expose the operating model that allows repeated failure to persist.
